First seen in Sabek, Masik reveals that the Eye of Ulos is one of six keys for opening Brizoor Waal;

“This is the Eye of Ulos. It opens a rift to Iktel, Plane of Frost. We must take it to Brizoor Waal to aid Azema’s return.”- Masik, Eye of Ulos (RPA-ADV4) 

When traveling to Iktel, Plane of Frost, the Eye becomes one with the statue of Uumloktesh;

Near the peak, the Eye of Ulos rolls off the trail and into deep snow. You follow it to a point just below the statue’s back…You also see a single empty socket in the statue’s elbow where, presumably, another eye should be. The Eye of Ulos is drawn to that space.

As you claim control over Brizoor Waal’s attachment to this plane, every eye in the statue's stone glows with renewed vigor. With a great rumble, the monument to Uumloktesh comes to life.
